NO PURCHASE OR BLOOD DONATION NECESSARY TO ENTER OR WIN A PRIZE IN THIS SWEEPSTAKES. A PURCHASE OR DONATION WILL NOT IMPROVE YOUR CHANCES OF WINNING. VOID WHERE PROHIBITED BY LAW.

AS A CONDITION OF PRIZE ACCEPTANCE, WINNERS WILL BE REQUIRED TO PROVIDE AND/OR COMPLETE AN AFFIDAVIT OF ELIGIBILITY, TAX INFORMATION, RELEASE OF LIABILITY, AND PRIZE ACCEPTANCE AGREEMENT, AS REQUESTED BY SPONSOR.

BY ENTERING (OR OTHERWISE PARTICIPATING) IN THE SWEEPSTAKES, YOU AGREE TO THESE OFFICIAL RULES, WHICH CREATE A CONTRACT, SO READ THEM CAREFULLY BEFORE ENTERING. WITHOUT LIMITATION, SUCH CONTRACT INCLUDES A CLASS ACTION WAIVER AND A LIMITATION OF YOUR RIGHTS AND REMEDIES.

1. SPONSOR: Versiti, Inc., 638 North 18th Street, Milwaukee, WI 53233.
2. ELIGIBILITY: Versiti’s Universal Donor Sweepstakes (the “Sweepstakes”) is open to legal U.S. residents who are 18 years of age or older at the time of entry and have reached the age of majority in their respective states of residence. Employees, officers, directors, agents, independent contractors and representatives of Sponsor and its respective parents, affiliates, subsidiaries, wholesalers, retailers, distributers, advertising and promotion agencies, and their immediate family members (mother, father, sister, brother, husband, wife, daughter, son, step-father, step-mother, step-daughter, step-son, step-brother, step-sister, half-brother, half-sister) and/or those living in the same household of each are not eligible. The Sweepstakes is subject to all applicable federal, state, and local laws and regulations. Void where prohibited or restricted by law or regulations. Participation constitutes entrant’s full and unconditional agreement to these terms and conditions and Sponsor’s decisions, which are final and binding in all matters related to the Sweepstakes.
3. SWEEPSTAKES PERIOD: The Sweepstakes begins at 12:01 a.m. Central (“CT”) on June 1, 2022 and ends at 11:59 p.m. (CT) on September 30, 2022.
4. HOW TO ENTER: The Sweepstakes consists of one (1) drawing. Eligible individuals may enter the Sweepstakes by using one of two methods outlined below. The choice of entry method will not affect an entrant’s chances of winning.
   1. To enter by making two donations of O-positive or O-negative blood: Donate or attempt to donate O-positive or O-negative blood at a Versiti Blood Center donor center or mobile drive (i) between June 1, 2022 and July, 31, 2022 and (ii) between August 1, 2022 and September 30, 2022. For locations of Versiti Blood Centers and mobile drives, please visit Versiti.org. Donors will be tracked by Sponsor, and those individuals who make a donation of O-positive or O-negative blood type during each of the two stated periods above will automatically receive one (1) entry into the drawing. To enter via this entry method, individuals must have O-positive or O-negative blood type and must meet the minimum qualifications to donate, which can be found at Versiti.org.
   2. To enter without donating (“AMOE”): To receive one (1) entry into the drawing without donating blood, please handwrite the following information on a 3”x5” card: the words “2022 Versiti’s Universal Donor Sweepstakes,” your first and last name, date of birth, address (no P.O. boxes accepted), city, state, zip code, telephone number, email address, and the words “I have read and accept the Official Rules.” Then mail the completed 3”x5” card in a stamped #10 business envelope to: Versiti, Inc., 2022 Universal Donor Sweepstakes, Attn: Donor Marketing, P.O. Box 2178, Milwaukee, WI 53201-2178. All mail-in entries must be postmarked by September 30, 2022 to be eligible. By submitting a mail-in entry, you will receive one (1) entry into the drawing specified on the mail-in entry card. Please note that there is no requirement with respect to blood type for AMOE entrants.

   Limit one (1) entry into the Sweepstakes, regardless of method of entry. Any attempt by an entrant to obtain more than the stated number of entries by using multiple/different addresses or any other methods will void that entrant’s entries and that entrant may be disqualified from the Sweepstakes. Entries submitted by anyone other than the entrant are void. Sponsor will not verify receipt of entries for entrants. All entries submitted become the sole property of Sponsor and will not be returned. Sponsor is not responsible for lost, late, misdirected, illegible, damaged, or postage-due mailed entries.

5. PRIZE AND APPROXIMATE RETAIL VALUE (“ARV”): One (1) potential winner will be randomly selected in total. The prize package is as follows:
   • Five-day/four-night trip for winner and three (3) guests to Universal Orlando in Orlando, Florida. Trip includes round-trip economy-class airfare, hotel accommodations, and 3-park/5-day park-to-park tickets. See details below.

   Travel Details: Prize package includes: (i) round-trip, economy-class airfare to and from Orlando, Florida for winner and three guests; (ii) four-night hotel accommodations (four-person occupancy room), and (iii) four 3-park/5-day park-to-park tickets for Universal Orlando, which includes Universal Studios Florida, Universal’s Islands of Adventure, and Universal’s Volcano Bay water theme park. If winner lives within a 150-mile radius of Orlando, Florida, ground transportation may be provided in lieu of air transportation at the sole discretion of the Sponsor, and no compensation or substitution will be provided for difference in prize value. Sponsor will make the sole determination whether winner’s residence is greater than 150 miles away from hotel location, using its own methods and tools (e.g., maps, GPS, mileage calculator). Winner and all guests must travel together on the same itinerary. If winner is unable to travel because of any COVID-19 related restrictions, including in relation to any requirement to be vaccinated, the prize will be forfeited. Guests must be at least 18 years of age or older, unless guest is a family member of the winner or unless winner obtains prior written consent from Sponsor to bring a guest who is less than 18 years of age and not a family member. At Sponsor’s discretion, guests may be required to execute liability/publicity releases prior to issuance of travel documents. Winner and guests must possess any necessary valid travel documents (e.g., valid government-issued photo ID). Travel restrictions, conditions, and limitations may apply. Winner must use the prize before December 15, 2023, or prize will be forfeited (at Sponsor’s discretion). Blackout dates for travel are November 20, 2022 – November 30, 2022, December 18, 2022 – January 6, 2023, March 15, 2023 – March 31, 2023, and November 19, 2023 – November 30, 2023. Air travel will be from a major domestic airport near winner’s place of residence as determine by Sponsor. Sponsor will determine airport, airline, flight itineraries, and hotel accommodations in its sole discretion. No refund or compensation will be made in the event of the cancellation or delay of the flight. No refunds or credit for changes are allowed. All other expenses and costs not expressly listed above, including but not limited to ground transportation, meals, incidentals, gratuities, passenger tariffs or duties, airline fees, surcharges, airport fees, service charges or facility charges, personal charges at lodging, security fees, taxes, or other expenses, are the winner’s sole responsibility. Travel restrictions may apply. The actual value of the prize is expected to vary based on the point of origination, airfare fluctuations, hotel pricing, and ticket pricing. If the actual value of the prize is less than stated, the difference will not be awarded in cash, and no such claim will be honored. Sponsor will not replace any lost, mutilated, or stolen tickets, travel vouchers, or certificates.

   Additional Prize Details: Winning a prize is contingent upon fulfilling all requirements set forth herein. PLEASE NOTE: All taxes on prizes are the sole responsibility of the winner. Any and all guarantees and warranties for prizes are subject to the service providers’ terms and conditions, and winner agrees to look solely to such service providers for any warranty or guarantee claim. Where not prohibited, affidavit of eligibility (including SSN), prize acceptance form, tax information, publicity release, and/or mutually acceptable liability releases may be required from winner and winner’s guests if requested by Sponsor. Prize is not transferable, and no substitution or cash equivalent is allowed, except in Sponsor’s sole discretion. Sponsor reserves the right to substitute a prize of the same approximate retail value if advertised prize becomes unavailable for any reason, including but not limited to travel restrictions due to COVID. Because the value of the prize exceeds $600, the winner’s social security number or taxpayer I.D. will be required in order to issue a Form 1099-MISC showing prize ARV as income to winner. Prize consists only of items specifically listed as part of the prize. All costs and expenses not specified herein relating to a prize are the sole responsibility of the winner.

6. ODDS OF WINNING, WINNER SELECTION, AND NOTIFICATION: Odds of winning depend on the number of eligible entries received. The potential winner will be selected randomly on or about October 6, 2022. Winner will be notified via phone and/or email on or about October 6, 2022 (using information in Sponsor’s records or information provided via the mail-in entry) to verify his/her eligibility and ability to accept the prize, and to obtain his/her complete contact information. Any such message will instruct the winner to reply as specified within five (5) full business days (measured in the local time zone) after the time that the message was left or sent. Winner must reply as instructed to identify him/herself, confirm his/her eligibility and ability to accept the prize, and provide his/her complete contact information within such five (5) full business day period, or he/she will be disqualified. Potential winner will subsequently be required to complete an affidavit/certification of eligibility, a liability release, and (where legal) a publicity release, which must be returned within the reasonable time period specified by Sponsor at the time of such request. Winner will also be required to return a properly completed IRS W-9 form within the time period specified by Sponsor, and each such winner will receive an IRS Form 1099 issued in his/her name for the fair market value of prize received. Proof of identity, age, and legal residence may be required to claim a prize. Winner may be disqualified in Sponsor’s sole discretion if: (a) he/she fails to reply or to provide any required information or properly completed documents within the required time period; (b) he/she fails to comply with any of the terms of these Official Rules; or (c) if any prize or prize notification is returned as undeliverable. In the event of disqualification of the winner, at Sponsor’s sole discretion the prize may be awarded to an alternate winner selected from among the remaining applicable eligible entries via the means of winner determination described herein, pending verification of eligibility. If any prize remains unclaimed after three (3) potential winners have been notified for that prize, the prize may remain unawarded, in Sponsor’s sole discretion. No responsibility is assumed by Sponsor for any incorrect or missing contact information associated with an entry, or for any change of contact information of entrant after entry submission. Upon winner confirmation, Sponsor will provide information to winner on prize fulfillment.
7. PUBLICITY RELEASE: Subject to applicable privacy laws and policies, acceptance of the prize constitutes consent to the use of the winner’s name, biographical data and likeness for advertising, promotion, or marketing purposes, as permitted by law, without additional compensation, by Sponsor.
8. RELEASE: By entering, entrant agrees to accept and abide by the Official Rules of this Sweepstakes and agrees that any dispute with regard to the conduct of this Sweepstakes, rule interpretation, or award of prize shall be submitted to Sponsor, whose decision shall be binding and final. By participating, entrant agrees to release and hold harmless Sponsor, participating dealers and retailers, affiliates, sales representatives, distributors and promotional and other vendor agencies, their respective parent companies, affiliates, subsidiaries, service agencies, independent contractors, and the officers, directors, employees, agents and representatives of any of the above organizations (“Releasees”) from any injury, loss or damage to person, including death, or property due in-whole or in-part, directly or indirectly, to the acceptance or use/misuse of a prize, participation in any Sweepstakes-related activity, or participation in the Sweepstakes. Entrant hereby agrees to defend, indemnify, and hold harmless Releasees against any and all third-party claims, damages, liabilities, costs and expenses, including reasonable legal fees and expenses, arising out of or related to any breach by the entrant of any provision of this document, including, without limitation, entrant’s representations and warranties.
9. ADDITIONAL RULES: Sponsor is not responsible for any typographical or other error in the printing of this offer, administration of the Sweepstakes, or in the announcement of a prize, including erroneous appearance of qualification for the prize, and under no circumstances will more than the stated number of prizes be awarded. Sponsor reserves the right, in its sole and absolute discretion, to modify, cancel, or suspend this Sweepstakes, or to amend the Official Rules at any time, without prior individual notice, should any factor interfere with the administration, security, or proper play of this Sweepstakes as contemplated by these Official Rules. If for any reason Sponsor is prevented from continuing with this Sweepstakes, or the integrity and/or feasibility of the Sweepstakes is severely undermined by any event including but not limited to fire, flood, epidemic, pandemic, earthquake, explosion, labor dispute or strike, act of God or public enemy, satellite or equipment failure, riot or civil disturbance, war (declared or undeclared), terrorist threat or activity, or any federal, state, or local government law, order, or regulation, or any order of any court or jurisdiction, Sponsor shall have the right, in its sole discretion, to abbreviate, modify, suspend, cancel, or terminate the Sweepstakes without further obligation. If Sponsor in its discretion elects to so abbreviate the Sweepstakes, Sponsor reserves the right, but not the obligation, to award the Prize from among all eligible entries received to date. All entries are the physical property of Sponsor and are not returnable.
10. DISPUTES/CHOICE OF LAW: EXCEPT WHERE PROHIBITED, EACH ENTRANT AGREES THAT: (1) ANY AND ALL DISPUTES, CLAIMS AND CAUSES OF ACTION ARISING OUT OF OR CONNECTED WITH THIS SWEEPSTAKES OR ANY PRIZE AWARDED SHALL BE RESOLVED INDIVIDUALLY, WITHOUT RESORT TO ANY FORM OF CLASS ACTION, AND EXCLUSIVELY BY STATE OR FEDERAL COURTS SITUATED IN WISCONSIN, (2) ANY AND ALL CLAIMS, JUDGMENTS, AND AWARDS SHALL BE LIMITED TO ACTUAL OUT-OF-POCKET COSTS INCURRED, BUT IN NO EVENT ATTORNEYS’ FEES, (3) NO PUNITIVE, INCIDENTAL, SPECIAL, CONSEQUENTIAL, OR OTHER DAMAGES, INCLUDING, WITHOUT LIMITATION, LOST PROFITS, MAY BE AWARDED (COLLECTIVELY, “SPECIAL DAMAGES”), AND (4) ENTRANT HEREBY WAIVES ALL RIGHTS TO CLAIM SPECIAL DAMAGES AND ALL RIGHTS TO SUCH DAMAGES MULTIPLIED OR INCREASED. WISCONSIN LAW, WITHOUT REFERENCE TO CHOICE OF LAW RULES, GOVERNS THE SWEEPSTAKES AND ALL ASPECTS RELATED THERETO.
11. PRIVACY: As a condition of entering the Sweepstakes, each entrant agrees that personally identifiable information submitted as part of an entry will be used to administer the Sweepstakes, select the winners, and fulfill the prize. Such information may be shared with Sponsor’s agents and affiliates as necessary to administer the Sweepstakes and to fulfill the prize. Information will be treated in accordance with Sponsor’s Privacy Policy available at www.versiti.org/privacy-policy.
12. WINNER'S LIST: To receive any legally-required name of the winner and/or a copy of these Official Rules, send a legal-size, self-addressed, stamped envelope within ninety (90) days of the end of the Sweepstakes to: 2022 Versiti’s Universal Donor Sweepstakes–Winner List, Attn: Donor Marketing, P.O. Box 2178, Milwaukee, WI 53201-2178.
